DIAGNÓSTICO DIFERENCIAL DE BACILOS GRAM POSITIVOS

RESUMO

Os bacilos são caracterizados pelas suas formas de bastonetes que são determinadas por sua parede celular e que na sua minoria são gram positivos. Possui sensibilidade á penicilina, sendo a maioria apresentada como anaeróbios facultativos. Podem ser divididos em regulares, corineformes, esporulados e ramificados. Dos regulares podemos citar o gênero Listeria spp onde a espécie de maior importância é a L. monocytogeneses que é contraída por ingestão de alimentos contaminados causando listeriose, podendo atingir meninges, olhos, coração, além de infecções neonatais. A sua identificação pode ser feita por testes de hemólise, CAMP teste, motilidade e prova de esculina. Outro gênero são os Erisipelotrix spp com a espécie E. thusiopathiae que possui maior impacto em suínos causando erisipela, já no homem causa lesões dolorosas podendo disseminar e causar endocardite, seu diagnóstico pode ser por suas características de ser não álcool-acido resistente, anaeróbia facultativa e não esporulada. Também tem a Kurthia que pode ser diferenciada em Lactobacillus spp que fazem parte da microbiota natural, podendo ser identificada por teste de catalase. No caso dos esporulados temos os Bacillus spp com suas espécies de maior importância que são os B. anthracis, B. cereus, B. subitilis, suas formas e localização de seus endósporos são uteis para classifica-los, a maioria é catalase positiva e podem apresentar capsula polipeptídica em presença de íons bicarbonato, além de testes como cor de suas colônias, motilidade, hemólise, citrato e lecitinase. Já os ramificados tem-se ao Actinomicetos com grande aplicação industrial tanto farmacêutica como alimentícia, no caso da Nocardia spp possuem mais de onze espécies e que podem causar pneumonia e lesões em pele, para diagnostica-las é possível fazer testes como de álcool-acido onde são resistentes. Ainda nos ramificados pode ser citado os genêros Rhodococcus spp e os Streptomyces spp onde o diagnóstico pode ser feito através de outros microrganismos e pelas hifas com colônias pigmentadas, respectivamente. Outros bacilos de grande importância são os corineformes com as espécies C. diphtheriae e C. pseudotuberculosis, esse gênero pode ser diferenciado por diversos testes como OF glicose, redução de nitrato, reação de CAMP, urease e utilização de diversos carboidratos. Outros gêneros são os Rothia spp que são diagnosticados por suas colônias brancas e rugosas além de fermentarem glicose e esculina, Arcanobacterium spp causadores de patogenia em orofaringe, cujo diagnósticos é feito pela sua formação de hemólise e por crescerem em CO2, a Gardnerella que pode ser identificada por testes como catalase, oxidase e anaerobiose e os Oerskovia spp que identifica-se pelo teste de catalase e por apresentar ramificações. Embora sejam testes diferenciados, todos seguem o mesmo padrão vindos de bacterioscopia e isolamento.

ABSTRACT

The bacilli are characterized by their forms of rods which are determined by their cell wall and that in his minority are gram positive. Has sensitivity to penicillin, mostly presented as facultative anaerobes. They can be divided into regular, coryneform, spoilage and branched. Regular can cite the genus Listeria spp where the species of greatest importance is the L. monocytogeneses that is contracted by eating food contaminated causing listeriosis, reaching meninges, eyes, heart, and neonatal infections. The identification can be made by hemolysis testing, CAMP test, esculin and motility test. Another genre are Erisipelotrix spp with E. thusiopathiae species that has the greatest impact in pigs causing erysipelas, as in man causes painful lesions can spread and cause endocarditis, diagnosis may be due to its characteristics of being non-alcohol resistant acid, facultative anaerobic and non-sporulated. It also has the Kurthia which can be differentiated Lactobacillus spp that are part of the natural microflora, and may be identified by catalase test. In case of spoilage have the Bacillus spp with their species of greatest importance are the B. anthracis, B. cereus, B. subitilis, its forms and location of their endospores are useful to classify them, most are catalase positive and can present polypeptide capsule in the presence of bicarbonate ions, and tests such as color of their colonies, motility, hemolysis, citrate and lecithinase. It has branched to the Actinomycetes with great industrial applicability both pharmaceutical and food, in the case of Nocardia spp have more than eleven species and can cause pneumonia and skin lesions, for diagnosing it is possible to make tests as acid-fast where they are tough. Still in branched may be mentioned the genres Rhodococcus spp and Streptomyces spp where the diagnosis can be made through other microorganisms and the hyphae with pigmented colonies, respectively. Other major importance are the coryneform rods with the species Corynebacterium diphtheriae and C. pseudotuberculosis, this genus can be distinguished by various tests such OF glucose, nitrate reduction, CAMP reaction, urease and utilization of various carbohydrates. Other genres are Rothia spp who are diagnosed by their white and wrinkled colonies addition to ferment glucose and esculin, Arcanobacterium causing spp pathogenesis in the oropharynx, whose diagnosis is done by training hemolysis and grow in CO2, which can be Gardnerella identified by tests such as catalase, oxidase and anaerobiosis and Oerskovia spp identifying by the catalase test and branching present. Although different tests, all follow the same pattern coming from Gram stain and isolation.
